![]() ![]() However, regional nodal status is the single most important prognostic characteristic of clinically localized MCC. This indicates that modified MMS can be applied to various regions of the body.Ī number of clinical and pathologic factors have been associated with a poor prognosis in MCC including male sex, older age, increased tumor size, and an immunocompromised state. Of the five patients who underwent modified MMS, three had MCC in the upper or lower extremity and two had MCC in the head and neck region. The observation that over half of the patients who underwent modified MMS remained well, while another died from an unrelated disease, indicates that modified MMS was superior to other surgical excision methods, at least within the limitations inherent to this study because of its small sample size. One died from a disease not related to MCC, and the other one was lost to follow-up. Of the five patients who underwent modified MMS, two remained well for 6 years or more postoperatively and one remained well through 14 months of follow-up. If the cancer extends beyond the resected tissue, additional excision is repeated until no more cancer cells are present in the margin of the surgically resected tissue. In this method, a plastic surgeon and a pathologist collaborate to perform margin-negative resection of cancer. In this study, modified MMS was performed in five patients, and was the main method for surgical excision. If the frozen biopsy results show malignancy, additional excision from that section is performed with the same procedure until cancer cells are no longer visible on the frozen biopsy. The resected section tissues are sent for frozen biopsy. All sections are then mapped and resected in order. The base of the lesion is also divided into sections, if necessary. The area between the previous resection margin and the new resection margin is divided into approximately 10 to 20 sections. ![]() In addition, a new resection margin is drawn with a width of 1–2 mm within the previous resection margin. First, WLE is performed as described above. Modified MMS combines the principles of MMS and WLE. In MMS, an average of 2–4 mm margin of clinically normal surrounding tissue is excised, and the procedure is continued until the absence of cancerous tissue is confirmed throughout the excision. WLE is an extensive excisional procedure with a 1- to 2-cm surgical resection margin that can be used for MCC treatment according to the guidelines of the National Comprehensive Cancer Network (NCCN). Our analysis investigated the frequency of each method and the final surgical margin. The surgical treatments included wide local excision (WLE), Mohs micrographic surgery (MMS), and modified MMS. However, no case studies of MCC in Asian patients have been reported since. A 2008 study presented 16 Japanese MCC patients, a 2010 study described seven Korean patients, and a 2012 study analyzed 22 Chinese patients. Although the reported incidence of MCC has increased by 8% per year between 19, cases in Asian patients have been rarely reported. Of these cases, 94.9% were white, 1.0% black, and 4.1% belonged to other races (Asian-Pacific Islander, American Indian, or other), indicating that MCC was very rare in Asian patients. According to the most recent available analysis, based on the 2011 Surveillance, Epidemiology, and End Results data in the United States, the incidence of MCC was 0.79 per 100,000. As mentioned earlier, MCC is more common in Caucasian patients, and its incidence varies across different regions of the world. The elderly and immunocompromised persons are at particularly high risk. MCC is very aggressive its mortality rate exceeds 33%, making it twice as lethal as malignant melanoma. It is rare, but is primarily found in older Caucasian patients. Merkel cell carcinoma (MCC) is a neuroendocrine trabecular cancer of the dermis, first described by Toker in 1972, that poses a significant risk of local invasion, lymph node metastasis, and distant metastasis. ![]()

![]() ![]() Lookout Mobile Security, now rebranded as Lookout Security and Antivirus or Lookout Personal, was one of the first Android antivirus apps, and for a long time the best. Read our full Avast Mobile Security review. Since the stand-alone price for SecureLine is $60 per year, paying an extra $20 on top of the premium Android antivirus tier for unlimited VPN data is not a bad deal. The built-in client for Avast's SecureLine VPN is just a tease, and the only way to use it is to pay for an Ultimate tier. Users of either paid plan also get additional anti-theft features, an app locker and tech support. Those ads go away if you pay for either of Avast Mobile Security's paid tiers, Premium ($2 monthly or $20 yearly) or Ultimate ($7 monthly or $40 yearly). (It's since been removed.) And the free version's ads and constant nags to upgrade are annoying and intrusive. Some of Avast's anti-theft functions didn't work for us, and its call-blocking feature didn't work at all. While Avast's malware protection is good, it's far from perfect. Read our full Norton Mobile Security review. It also checks apps in the Google Play store for security and privacy risks even before you install them. Norton's killer feature is the unique App Advisor, which checks apps on your device for heavy data usage and unusual behavior. Norton also killed Norton Mobile Security's free tier, which was the best among all the Android antivirus apps we'd recently tested. Unfortunately, it no longer has anti-theft functions, nor its contacts backups and Link Guard malicious-link blocker. Like Droid Optimizer, there's also a ranking system to encourage regular use.Norton Mobile Security, aka Norton 360, offers the best malware protection of any of the Android antivirus apps we tested. As its name promises, All-In-One Toolbox tries to do it all. If that wasn't enough, there's also a mass app deleter, advanced file cleaning tools, and Wi-Fi analysis. If you're running out of juice, the Battery Saver section is another background task killer (see below for a warning on this), but it also provides your current battery statistics. You can set this automatically with the Boost+ function if you'd prefer, but doing so requires an in-app purchase. The Boost function cleans your system cache and closes background apps, again with two taps. It's a similar process with other areas of the app. It just takes one tap to analyze your device, then another tap to delete, and you're done. It's capable of cleaning temporary files on your Android device, wiping your cache, and deleting empty folders and orphaned files. What about monitoring your battery health, your CPU temperature, and those pesky mobile ads? The appropriately named All-In-One Toolbox is here to do it all. It also comes with a "good night scheduler" to conserve energy, automatically disabling features like Wi-Fi when no connections are active.Ĭleaning your phone of unnecessary files is only one part of the maintenance task. Oh, and your one, single editor at an imprint? They have a team. have far fewer editors available, making it much riskier and significantly more difficult to land a book deal. There are approximately 40 editors in the total publishing pool (one per imprint) where an agent can send your manuscript before the submission process is exhausted, although some genres like sci-fi, or specific markets like the U.K. This is known as sending out in “rounds”. Some will send to a handful, wait for feedback, then move on to the next few. Some literary agents send to all editors at once, hoping to drive up interest with a “big bang” effect. Once your literary agent has her pitch letter and her list (she may or may not share either of these with you, or she may even ask for your input in writing her pitch letter, which will look eerily similar to your original query letter), she will send your manuscript out into the world. They’re extremely adept at literary matchmaking. And, by the way, if you’re still unconvinced that you need a literary agent to secure a book deal, one of the best things about agents is that it’s their job to find out exactly what editors are looking for and which genres they represent. Your literary agent can only send to one person per imprint, so she’ll likely spend a while refining her list, ensuring that she’s aligning your manuscript to an editor’s tastes. Essentially, she will do whatever she can to hype it up, making it irresistible to editors. And, in some cases, she may even get a few book blurbs before she takes it out, especially if you happen to know any big-named authors who are willing to vouch for you. She may even make a few phone calls, generating hype around the “launch”. Your literary agent will prepare a pitch letter to send to editors within publishing houses and imprints. In other words, your baby is about to be sent out to acquisitions editors at publishing houses who are simply going to LOVE it. Your literary agent will have worked with you to give your manuscript the best possible chance of success, as she will know better than anyone that it all comes down to this: your shot at a traditional book deal. You’ve signed with a literary agent, been back and forth several times on your manuscript, editing and polishing it to bulletproof perfection. If you have reached the on submission stage: congratulations. What does it mean to be on submission to publishers? Nothing quite compares to this journey, and every author’s journey is unique. Staying sane, and having an excellent support network around you (plus chocolate chip cookies and/or Ben & Jerry’s) as you navigate being on submission to publishers is crucial. And the timeline – more on that below – can be completely bananas. Personally, I didn’t sleep well for the first couple of weeks. Writer imposter syndrome inevitably creeps in. At this point, there’s no rhyme or reason to doubt that your book is good enough to be published, and yet, this is inevitably what you start doing: doubting yourself. Having personally been out on submission once, and now about to go out for a second time, I can tell you, it is truly agonizing. Publishing is an incestuous industry, and news travels fast. So it’s fine to say you’re out on sub, but not to say who’s reading it. ![]() It sort of ruins the game – much like you wouldn’t tell one company at a job interview all of the other companies you’re interviewing with. However, a more probable reason is that other editors shouldn’t be able to easily locate who else is reading it. ![]() And yet, being on submission is how book deals happen! It’s the end game to getting your book published! So why aren’t we talking about it more? My main theory is because money is involved, and it’s uncouth to talk about money. It’s like this great, mystical process that is totally overshadowed by the importance of securing a literary agent. The secret of being “on sub”įor reasons unknown, being out on sub is not something that’s commonly spoken about by writers. Forget any semblance of control being out on submission (or “on sub”) is a wild experience, and one that’s totally out of your hands. Plus, it’s totally driven by publishers: the gatekeepers to your publishing dreams. That’s because going out “On Submission to Publishers” is such a different beast than just subbing a short story to Submittable. You’ll notice this posted under the Publishers topic rather than the Submissions topic. ![]() ![]() ![]() Siyuan Li, Lu Lu, Zhiqiang Zhang, Xin Cheng, Kepeng Xu, Wenxin Yu. Video keywords are especially important to consider if you don't have many subscribers as you will be relying heavily on search behavior on YouTube itself or Google for users to find your content. Of course there are many other ranking signals including the quality of the video which YouTube infers from the click through rate, watch duration and engagement. Some of the most important ones are the video title, video description, video tags and the channel that publishes it.
